Thank you for signing up for the Jumpstart Program.

Here’s what happens now:

1. The Moday Center office manager will reach out to you regarding completing electronic intake paperwork, scheduling information, and to get your preferred address within 24 hours (Monday – Thursday) or the following Monday (if Friday, Saturday, or Sunday).**IMPORTANT: Please add office@modaycenter.com to your “Safe Sender” email list to ensure you receive all communication from us.**
2. Your test kits will be ordered and shipped directly to your home. The urine adrenals test is completed at home and takes 14 business days for us to get the results. I will highlight all the areas you will need to fill out on the intake form. You will also be shipped a micronutrient kit box along with requisition. There is nothing for you to do with these, you will just hand them to the phlebotomist. For the micronutrient blood draw, I will also include that information in the box. There is a location near Washington Sq. which draws this for free as well as a mobile phlebotomy company which also draws this for free. There is no fasting for this test.
3. Once you set up your draw date/know when you can complete the tests, we will schedule you for both your medical and your nutrition appointment. These can be either spread one week apart or back to back. They will need to be completed within two months of signing up. They will be phone or via Zoom. Generally, it is advised to spread them out since you get a lot of information.
