Thank you for signing up for the Immunity Plus Program.

 

Here’s what happens now:

  1. The Moday Center office manager will reach out to you regarding completing electronic intake paperwork, scheduling information, and to get your preferred address within 24 hours (Monday – Thursday) or the following Monday (if Friday, Saturday, or Sunday).**IMPORTANT: Please add office@modaycenter.com to your “Safe Sender” email list to ensure you receive all communication from us.**
  2. We will schedule you for both your medical and your nutrition appointment. These can be either spread one week apart or back to back. They will need to be completed within two months of signing up. They will be phone or via Zoom.
  3. Your test kit will be ordered from Cyrex and shipped directly to your home. Any questions regarding tracking can be directed to the lab at (toll-free) 877-772-9739. The kit will be shipped via USPS and should be received within 3-5 business days.
  4. Once you receive the test and ship it back, the test results will be sent to the office within 14 business days.
