Thank you for your interest in becoming a patient of Dr. Moday. We are happy you are making this first step. In order to make an initial appointment, please read our payment/insurance policies first, and then follow the steps below if you would like to move forward.
Step #1
Your first step is to register as a new patient by filling out this registration form. Once you have created your account, you will need to verify your email address by clicking on a link in an email that will be sent to you, and then you will be able to log on to our secure patient portal using the username and password you selected during the registration process. There you will be able to see our openings and Request an Appointment online. We can generally get you in within a week or so of your registration, so rest assured there isn’t a long wait. Our office staff will confirm your appointment time, after which a $150.00 deposit will be charged to your credit card on file to hold your initial consultation appointment. This $150.00 deposit will be applied toward charges incurred during your first appointment, and is not the full cost of your first appointment. The deposit is fully refundable unless you miss your appointment or fail to cancel your appointment within 48 hours.
Step #2
After your initial consultation appointment is confirmed, make sure to fill out the 6 intake questionnaires in the Questionnaires section of your patient portal. (The registration form was not the only intake form.) These are fillable forms and will be submitted electronically to us. They cannot be filled out on a mobile/tablet device as the forms only work on computer browsers. The forms are fairly extensive, so please give yourself enough time to fill it out as completely as possible to get the most out of your evaluation. These will need to be received by us at least 24 hours prior to your appointment. If your appointment is on a Monday it must be received by Friday. These cannot be filled out on the day of your appointment.
Step #3
Collect any recent lab results, printed copies of radiology reports (no CDs please) or medical records you have pertaining to your condition and upload them as PDFs to your patient portal account, or bring them with you to your first appointment. You can also have your physician fax records to us at (215) 515-0033.
What is included in your initial consultation:
- A comprehensive one-on-one visit with Dr. Moday
- Review of any previous relevant medical records and laboratory results
- Physical exam
- BIA (Body impedance analysis) in applicable cases
- Review of nutrition history and goals
- Development of a customized treatment plan including laboratory tests and review of recommended treatments
There are no other patients scheduled during your appointment slot.
