New Patients

Thank you for your interest in becoming a patient of Dr. Moday. We are happy you are making this first step.

In order to make an initial appointment, we ask that you read the following instructions carefully.

Step #1
To schedule an appointment with Dr. Moday, please go to our secure patient portal under the patient tab. Fill out the registration form completely. A $150.00 deposit will be charged to your credit card on file to hold your initial consultation appointment. This $150.00 deposit will be applied toward the office visit charges incurred during your first appointment, and/or any labs ordered by Dr. Moday. The deposit is fully refundable unless you miss your appointment or fail to cancel your appointment within 48 hours. Once you have registered, log into your patient portal and go to the Request Appointment area to select a day and time that you would like to be seen; someone will confirm your appointment with you via email or phone.

Step #2
After your initial consultation appointment is confirmed, make sure to fill out the new patient questionnaires in the Questionnaires section of your patient portal. These are fillable forms and will be submitted electronically to us. These are very extensive, so please give yourself enough time to fill it out as completely as possible to get the most out of your evaluation. These will need to be received by us at least 48 hours prior to your appointment. If your appointment is on a Monday it must be received by Friday. These cannot be filled out on the day of your appointment.

Step #3
Collect any previous lab results, printed copies of radiology reports (no CDs please) or medical records you have pertaining to your condition and bring those with you. You can also have your physician fax records to us at 215-515-0033 or you can upload any documents from your computer to your patient portal.

What is included in your initial consultation:

  • A 90-minute one-on-one visit with Dr. Moday
  • Review of any previous relevant medical records and laboratory results
  • Physical exam
  • BIA (Body impedance analysis) in applicable cases
  • Review of nutrition history and goals
  • Development of a customized treatment plan including laboratory tests and review of recommended treatments

There are no other patients scheduled during your appointment slot.

Generally patients will require 2-4 follow-up visits depending on the case and course of treatment.
