Scheduling your first appointment
Because we practice medicine outside of the conventional paradigm we require all potential new members learn a little more about who we are before scheduling their first wellness consultation. Functional medicine requires a completely different approach to health and wellness. With that said, please click here to learn how to schedule your new patient wellness consultation.
Payments
At the conclusion of your new patient wellness consultation (don’t have one scheduled? click here.) Dr. Moday will provide you with a membership or comprehensive wellness program price based on your health goals and needs. Prices are quoted individually as functional medicine is never a one-size-fits-all approach. The Moday Center does not bill insurance for services rendered. You will be provided with a “superbill” to submit to your insurance for reimbursement if available through your personal policy. We also do not participate with Medicare and we cannot bill Medicare for any services performed at the Moday Center. Please see out Insurance Policy for more information.
Do you offer payment plans?
Yes. We offer payment plans for both our membership and comprehensive programs. Feel free to inquire about these options before your appointment or after your wellness consultation.
Where are you located?
We are located at 110 S. 20th street in the Rittenhouse square neighborhood of Center City Philadelphia. Our building is between Chestnut and Sansom streets and our suite is on the 4th floor. There is an elevator on the left when you enter the lobby. To call the elevator, press the button and then pull the elevator door open, then the elevator gate. There is a light switch in the elevator that may need to be turned on. Once in, press 4 and the elevator will take you up to our reception area. The elevator door is slightly heavy so if you need any assistance, please call. You are also welcome to take the stairs located just past the elevator.
Parking and public transportation:
There are numerous parking facilities in the immediate vicinity including Park America at 2027 Sansom St., EZ-Park at 109 S. 20th street, and Allied parking on the corner of 20th and Sansom. There is 2 and 3-hour street parking available as well.
We are accessible by the Septa bus routes 9,12,17,21 and 42 amongst others. We are a 5-block walk from Suburban train station and quick cab ride away from 30th street station.
Can I bring a friend or family member to the appointment?
Absolutely, we encourage you to take notes as well. However we ask you to find childcare for any small children or infants as the appointments are lengthy.
What are your office hours?
Mondays-Thursdays 9am-5pm by appointment only.
Can Dr. Moday be my primary doctor?
No, we do not function as a primary care provider and recommend that you keep a relationship with a primary doctor for sick visits and regular screening exams. We are more than happy to work along side them as a consultant.
How many visits will I need?
This depends on the complexity of your problem. We require at least a 6 month engagement, as we understand there needs to be significant dietary and lifestyle changes and time for treatments to result in significant change. Most clients work with us for 12 months or more, or 3-6 visits.

Dr. Moday’s Practice Tenets
I understand that Dr. Moday is not a primary care doctor and does not provide primary care services. You should maintain an active relationship with a primary care provider in order to insure that those needs are met. Dr. Moday does not admit to any hospital or perform any consultations within any healthcare facility outside of the Moday Center.
The approach of Integrative Medicine and Functional Medicine addresses the underlying causes of disease and uses nutrition, and lifestyle interventions to manage medical problems. We use interventions based on medical literature. However as with any medical or health intervention, there are no guarantees made as to the results of the examinations, tests, diagnoses, or treatment by Dr. Moday/Moday Center.
Dr. Moday does not provide disability evaluations. She does fill out FMLA (family medical and disability Act) forms, or if a letter is required for other reasons, such as medical necessity, these will be charged at $25.00 per letter.
Dr. Moday does not generally use prescription drugs for treatment of her patients. In rare cases when a prescription medication has been written, such as a compounded medication, please allow 24 hours for the medication to be called or faxed in. If the request is made over the weekend the requests will be responded to on Monday morning when the office opens.
Dr. Moday will not fill any prescription medications prescribed by another physician. These should also not be discontinued without first discussing with either Dr. Moday or the prescribing physician and fully understanding the risks and benefits of such an action.
I fully understand and accept the above listed policies, and I do hereby voluntarily consent to evaluation and care with the Moday Center including physical exam and any mutually agreed upon diagnostic testing and treatment recommendation.
